What is my chair!

I found this chair at a garage sale and since it has been my favorite piece of furniture along with every one who sees it. The sad part is I don't know where it's from or anything about it.

Could anyone help id it for me? A water main just broke in my apartment and I need to know a value of it for insurance purposes and have no clue.

It's a great chair, it is kind of modern in style with one arm and a matching throw pillow in a green slightly patterned fabric and it swivels. Any help would be great.
